Overview
Welcome to Myoform. The terms "Myoform", "we", "us" and "our" refer to Myoform. We operate this website and store, including all related information, content, features, tools, products and services (together, the "Services"). Myoform is powered by Shopify, which enables us to provide the Services to you.
These Terms of Service ("Terms"), together with any policies referenced in them (including our Privacy Policy and Return & Refund Policy), set out your rights and responsibilities when you use the Services.
By visiting, browsing, interacting with, or purchasing through the Services, you agree to be bound by these Terms and our Privacy Policy. If you do not agree, you must not use the Services.
Section 1 — Eligibility, Access & Account
1.1 Age requirement
The Services are intended for adults. By using the Services, you confirm you are 18 years of age or older and have the legal capacity to enter into a contract.
1.2 Account information
You may be asked to provide information such as your email address, billing, payment, delivery address, and other details required to provide the Services. You confirm the information you provide is accurate, current, and complete, and that you have the right to provide it.
1.3 Account security
You are responsible for maintaining the confidentiality of your account credentials and for all activity under your account. You must notify us promptly if you suspect unauthorised access.
Section 2 — Myoform Products & Personalisation
2.1 Personalised, made-to-order products
Myoform offers supplements and related products that are personalised and made to order using your submitted information, which may include DNA-based insights and other personal inputs. Because each product is made for a specific individual, it cannot be resold or reused.
2.2 Genetic data
Where you provide a DNA sample or genetic data as part of the personalisation process, that data is treated as special category personal data and handled with the highest level of care. Collection, use, storage, and sharing of your genetic data is governed by our Privacy Policy, which you should read carefully before using the Services. By submitting genetic data, you acknowledge and consent to its use for the purpose of generating your personalised supplement formulation and associated health insights.
2.3 No medical advice
Myoform provides information intended for general wellness and education only. Nothing on the Services is medical advice and Myoform products are not intended to diagnose, treat, cure, or prevent any disease. If you have a medical condition, are taking medication, are pregnant or breastfeeding, or have concerns about suitability, you should consult a qualified healthcare professional before use.
2.4 Individual results vary
Any references to benefits or outcomes are not guarantees. Individual responses can vary based on many factors (including diet, lifestyle, training, sleep, and baseline health).
2.5 Product presentation
We make reasonable efforts to display products accurately, but packaging, colours, and appearance may vary by device and may change over time. Descriptions are subject to change without notice.
Section 3 — Orders, Acceptance & Cancellations
3.1 Order offer and acceptance
When you place an order, you are making an offer to purchase. We may accept or decline any order at our discretion. An order is not accepted until we confirm acceptance (for example, by email confirmation and/or dispatch confirmation), and payment has been received and processed.
3.2 Review before you buy
Please check your order details carefully before submitting. Because Myoform products are personalised and made to order, we may be unable to accommodate changes or cancellations once processing/production begins.
3.3 Cancellations
If you need to cancel, contact us immediately at support@myoform.io.

If your order has not entered processing/production, we may be able to cancel and refund it.

If processing/production has started (or a personalised product has been created), we cannot cancel or refund.
3.4 Personal use only
Purchases are for personal or household use only and not for commercial resale or export.
Section 4 — Prices, Taxes & Billing
4.1 Pricing
Prices, promotions, and availability may change without notice. The price charged will be the price shown at checkout at the time you place your order.
4.2 Taxes and delivery
Unless stated otherwise, prices may not include taxes, shipping, customs, or import charges, which may be applied depending on destination and local rules.
4.3 Payment details
You agree to provide valid, current payment information and authorise us (and our payment processors) to charge your payment method for the total amount shown at checkout.
Section 5 — Shipping, Delivery & Risk
5.1 Estimated delivery times
Delivery dates are estimates and not guaranteed. Delays can occur due to couriers, customs, or events outside our control.
5.2 Risk during delivery (consumer orders)
If you are a consumer, the goods remain at our risk until they are delivered to you (or to a person you nominate to receive them).
5.3 Lost or missing deliveries
If tracking shows "delivered" but you have not received your order, or if it appears lost in transit, contact us promptly. We will investigate with the courier and, where appropriate, arrange a replacement.
Section 6 — Returns, Refunds & Exchanges
6.1 Policy applies
Returns, refunds, and exchanges are governed by our Return & Refund Policy, which is incorporated into these Terms.
6.2 Personalised products are final sale
Because Myoform products are personalised and made to order, all sales are final and we do not accept returns or "change of mind" refunds for non-fault reasons.
6.3 Damaged, defective, or incorrect items
If your order arrives damaged, defective, or incorrect, contact us within 7 days of delivery and we will make it right (including replacement, reshipment, or refund depending on the issue).
6.4 No exchanges (generally)
Exchanges are generally not available due to personalisation. If we made an error, we will correct it.
6.5 Statutory rights
Nothing in these Terms limits your statutory rights. For EU deliveries, the 14-day cooling-off period may not apply to goods that are made to your specifications or clearly personalised. If your order is shipped into the European Union and you believe your situation qualifies for cancellation under applicable consumer law, please contact us and we will review it fairly.
Section 7 — Your Responsibilities
7.1 Follow instructions
You agree to follow any product instructions provided with your order and not to misuse the products.
7.2 Stop use if adverse effects
If you experience an adverse reaction, discontinue use and seek medical advice if needed.
7.3 Accurate information
You are responsible for ensuring the information you provide is accurate. If you provide incorrect details (such as address or personal inputs) this can affect the product and delivery, and may limit our ability to help.
Section 8 — Intellectual Property
All content on the Services (including trademarks, logos, text, graphics, images, videos, and design) is owned by Myoform or its licensors and is protected by applicable intellectual property laws. You may use the Services for personal, non-commercial use only and must not copy, reproduce, distribute, or exploit any content without written permission.
Section 9 — Optional Tools & Third-Party Links
We may provide access to tools or links operated by third parties. We do not control and are not responsible for third-party content, services, or practices. Your use of third-party services is at your own risk and subject to their terms.
Section 10 — Relationship with Shopify
Myoform is powered by Shopify, which provides the e-commerce platform. Purchases made through the store are made directly with Myoform. Shopify is not responsible for any aspect of sales between you and Myoform.
Section 11 — Privacy & Data
Your personal information, including any genetic or health-related data, is handled in accordance with our Privacy Policy. By using the Services, you consent to the collection, use, and sharing of information as described in the Privacy Policy, including sharing with Shopify and service providers that help us operate the Services. Given the sensitive nature of genetic data processed by Myoform, we strongly encourage you to read the Privacy Policy before submitting any biological or health information.
Section 12 — Feedback
If you submit feedback, reviews, suggestions, or other content ("Feedback"), you grant Myoform a perpetual, worldwide, royalty-free licence to use it for operating, improving, and promoting the Services. You confirm you have the rights to submit the Feedback.
Section 13 — Anonymised Data & Research Use
We may use aggregated and anonymised data derived from our users’ inputs — including genetic, health, and lifestyle data — for internal research purposes and to enhance the quality, accuracy, and personalisation of the insights we provide. This data is processed in a form that cannot identify any individual. It is not sold, shared with third parties for commercial purposes, or used in any way that would allow a user to be identified. Any research use of this kind is conducted in accordance with our Privacy Policy.
Section 14 — Errors, Inaccuracies & Updates
We may correct errors or inaccuracies and update information at any time without prior notice, including after an order is submitted. If we cancel an order due to an error, we will attempt to notify you and refund any payment received.
Section 15 — Prohibited Uses
You may use the Services only for lawful purposes. You must not misuse the Services, attempt to gain unauthorised access, interfere with security features, scrape content, use automated tools to access the Services, or engage in conduct that harms Myoform, Shopify, or other users.
Section 16 — Termination
We may suspend or terminate your access to the Services at any time if we believe you have violated these Terms. Obligations and provisions that by their nature should survive termination will survive (including IP, disclaimers, limitation of liability, and indemnity).
Section 17 — Disclaimers
The Services and products are provided "as is" and "as available", to the fullest extent permitted by law. We do not guarantee uninterrupted access or that content will always be accurate or complete. This section does not affect any rights that cannot be excluded under applicable law.
Section 18 — Limitation of Liability
To the fullest extent permitted by law, Myoform (and its directors, officers, employees, affiliates, and service providers) will not be liable for indirect, incidental, special, consequential, or punitive damages, or for loss of profits, revenue, data, or goodwill, arising from your use of the Services or products.
Nothing in these Terms excludes or limits liability for death or personal injury caused by negligence, fraud, or any liability that cannot be limited under applicable law.
Section 19 — Indemnity
You agree to indemnify and hold harmless Myoform, Shopify, and their affiliates and service providers from claims, losses, liabilities, and expenses arising out of your breach of these Terms, your misuse of the Services, or your violation of any law or third-party rights.
Section 20 — Severability
If any provision of these Terms is held to be unlawful or unenforceable, the remainder will remain in effect.
Section 21 — Entire Agreement
These Terms and the policies referenced in them constitute the entire agreement between you and Myoform regarding the Services and supersede prior agreements and communications.
Section 22 — Assignment
You may not assign your rights or obligations under these Terms without our written consent. We may assign our rights and obligations under these Terms as part of a business transfer or restructuring.
Section 23 — Governing Law & Jurisdiction
These Terms are governed by the laws of England and Wales. The courts of England and Wales will have exclusive jurisdiction to resolve disputes, unless mandatory consumer protection rules in your country of residence require otherwise.
Section 24 — Changes to These Terms
We may update these Terms from time to time by posting a new version on this page. If changes are material, we will provide notice where required by law. Your continued use of the Services after changes take effect means you accept the updated Terms.
